A 1.5-inch segment of around 200 hair strands (roughly the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the optimal sample size for screening and verification. For EtG, additional checks, and/or panels exceeding 10, 150mg of the sample is advisable. We suggest using a jeweler’s scale for specimen weighing. Should scalp hair be unavailable, an equivalent quantity of body hair can be sourced. By head hair, we mean only scalp hair. All other hair, such as facial and body hair, falls under body hair.

Process Overview

The laboratory processing of a drug test result involves four key stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning begins with the initial entry of a sample into a lab’s system. This step involves ensuring the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and addressing any extra data entry not provided by an electronic chain of custody system.

Screening entails an initial rapid assessment for drug misuse. Although Screening is cost-effective for dismissing drug use in most samples, confirmation is necessary for any positive screen to be legally valid. Samples that yield a presumptive positive require subsequent confirmation.

Should a sample provide a presumptive positive in Screening, further hair is extracted from the initial sample for the Extraction phase. Here, drugs are drawn from hair at much lower concentrations compared to methods like urine or oral fluid testing, making hair drug screening the most complex methodology.

Confirmation of positive screening results utilizes GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples undergo washing before confirmation as necessary. The comprehensive lab process from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.
