Process Overview

The four primary steps in the laboratory evaluation of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the preliminary handling of a sample within a laboratory’s system. This process verifies that the sample was correctly sealed and dispatched, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. While Screening is a cost-efficient method to dismiss drug usage on the majority of samples, any positive screen should be validated for it to be admissible in court. Samples that are preliminarily positive in Screening necessitate additional confirmation.

For samples preliminarily positive in the Screening stage, more hair is extracted from the original specimen and prepared for Extraction. Here, drugs are isolated from hair at much lesser concentrations compared to other methodologies (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Verification of any positive screening result is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing as required before confirmation. The whole lab procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and adherence to ISO / IEC 17025 criteria.
