Process Overview

The laboratory processing of drug test results encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, samples are initially processed into the laboratory’s system, ensuring proper sealing and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing necessary data entry absent from electronic chain of custody systems.

Screening consists of a preliminary assessment for illicit substances. Although Screening is a cost-effective way to exclude drug usage for most samples, positive outcomes require confirmation for courtroom validity. Presumptive positive results in Screening necessitate further confirmation.

For a presumptive positive sample in Screening, additional hair from the initial sample is taken for Extraction, where drugs are isolated from hair at lower concentrations than other methods (e.g., urine or oral fluid), making hair drug analysis particularly challenging.

Positive screening outcomes are confirmed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo washing as required before confirmation. The end-to-end process from Accessioning to Confirmation undergoes review in accordance with the CAP (College of American Pathologists) Hair guidelines and ISO / IEC 17025 accreditation.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
