When gathered close to the scalp, hair offers a detection timeframe of up to roughly 3 months for alcohol and drugs. Hair collection is straightforward, relatively hard to tamper with, and simple to transport.

A 1.5-inch sample comprising approximately 200 hair strands (akin to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, an optimal quantity for screening and confirmation purposes. For EtG, extended panels, and/or tests exceeding 10 panels, a specimen of 150mg is advised. We propose using a jeweler’s scale to weigh the sample. If scalp hair is unavailable, an equivalent volume of body hair can be utilized. When we refer to head hair, we mean scalp hair exclusively. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four fundamental stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to introducing a sample into the laboratory’s system. This step verifies proper sealing and shipping, assigns a LAN (Laboratory Accessioning Number), and completes any extra data entry beyond that provided by an electronic chain of custody system.

Screening entails an initial rapid examination for drug traces. Although Screening is a cost-efficient method to rule out drug usage in most samples, a positive screen necessitates confirmation to be legally admissible. Samples that test presumptively positive in Screening require a secondary verification.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original specimen for the Extraction process. During this stage, drugs are drawn out from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), which positions hair drug screening as the most challenging methodology.

Confirmation of positive screening outcomes is achieved via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing prior to confirmation as needed. The complete laboratory process, from Accessioning to Confirmation, is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
