A 1.5-inch portion consisting of roughly 200 hair strands (approximately the width of a #2 pencil) nearest to the scalp will produce 100mg of hair, which is the ideal amount for both screening and verification. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg of hair is advised. It's suggested to weigh the specimen with a jeweler’s scale. If scalp hair isn't available, an equivalent volume of body hair may be gathered. References to head hair denote scalp hair only. Body hair pertains to all other hair categories (e.g., facial, axillary, etc.).

Process Overview

The four key stages in the laboratory procedures for a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the initial incorporation of a sample into a lab's framework. This phase ensures the sample was adequately sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic custody system.

Screening entails a preliminary and rapid evaluation for drug abuse. Although Screening is an economical approach to exclude drug usage for most samples, a positive result requires confirmation to be valid in legal settings. Samples that test positive in Screening necessitate a secondary confirmation.

If a sample appears positive during the Screening phase, additional hair is extracted from the initial sample for Extraction preparation. In this phase, drugs are separated from hair at significantly lower concentrations than other methods (such as urine or saliva), making hair drug screening one of the most complex methodologies.

Confirmation of any positive screening outcome is achieved using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumed positive samples are cleansed before confirmation as needed. The full lab procedure from Accessioning to Confirmation is evaluated following the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.
