In the absence of scalp hair, an equivalent quantity of body hair is acceptable. The term "head hair" specifically pertains to scalp hair, whereas "body hair" encompasses other types (facial, axillary, etc.).

Process Overview

Laboratory processing of drug test results involves four primary phases: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample undergoes initial processing into the laboratory's system. This phase ensures the sample's secure sealing and shipment, allocates a random Laboratory Accessioning Number (LAN), and completes any pending data entry not covered by an electronic chain of custody platform.

Screening performs a preliminary evaluation for drug abuse. Economical in nature, Screening can preclude drug usage in a majority of cases; nonetheless, only confirmed positive results hold legal validity. Presumptive positives in Screening necessitate further confirmation.

Samples identified as presumptive positives in Screening require additional hair extraction and preparation for the Extraction phase. This stage extracts drugs from hair at a notably lower concentration than alternative methods (e.g., urine or oral fluid), highlighting the complexity of hair drug screening.

Confirmation for any positive screening results employs GC/MS, GC/MS/MS or LC/MS/MS techniques. Prior to confirmation, samples deemed presumptive positives are appropriately cleansed as required. The comprehensive process from Accessioning to Confirmation falls under the strict scrutiny of both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
