What it is, How it works

A 1.5-inch section of roughly 200 hair strands (approximately the thickness of a #2 pencil) near the scalp provides 100mg, the preferred sample size for both screening and confirmation. For EtG, additional tests, or those exceeding 10 panels, a specimen of 150mg is advised. Using a jeweler's scale to weigh the sample is recommended. If scalp hair is unavailable, an equivalent amount of body hair can be used. Head hair refers exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails adding a sample to the lab’s system. This step checks that the sample was sealed and shipped correctly, assigns a random LAN (Laboratory Accessioning Number), and handles any extra data entry not covered by an electronic chain of custody system.

Screening offers an initial rapid assessment for drug use. Although Screening is a cost-efficient way to eliminate drug usage in most samples, a positive result must be confirmed to hold up in court. Samples showing presumptive positivity in Screening require further confirmation.

If a sample shows presumptive positivity during Screening, more hair is taken from the original specimen for Extraction. In this phase, drugs are isolated from hair at much lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening the most challenging methodology.

Confirmation for any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation if required. The complete laboratory process from Accessioning to Confirmation is reviewed under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standard accreditation.
