What it is, How it works

When gathered near the scalp, hair can extend up to roughly a 3-month detection window for substances. Hair is uncomplicated to gather, moderately challenging to tamper with, and straightforward to transport.

A 1.5-inch segment of approximately 200 hair strands (comparable to the size of a #2 pencil) near the scalp will yield 100mg of hair, the optimum sample for screening and validation. For EtG, add-ons, and/or evaluations above 10 panels, 150mg of the specimen is advisable. It is advised to weigh the specimen using a jeweler’s scale. If scalp hair is lacking, a similar volume of body hair may be collected. When mentioning head hair, it specifically refers to scalp hair. Body hair pertains to all other varieties (facial, axillary, etc.).

Process Overview

The four primary phases in the laboratory processing of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial incorporation of a sample into a laboratory’s system. This incorporates verifying that the sample was properly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and completing any supplementary data not provided by an electronic chain of custody framework.

Screening involves an initial rapid examination for narcotics. Although Screening is a cost-efficient approach to exclude drug usage for most samples, a positive screen must be confirmed for it to hold up in court. Any samples that are preliminarily positive in Screening need a subsequent confirmation.

If a specimen shows a preliminary positive in the Screening stage, more hair is extracted from the initial sample and readied for Extraction. In this phase, drugs are separated from hair at a significantly lower concentration compared to other methods (e.g., urine or oral fluid), making hair drug screening one of the most challenging methodologies to execute.

Confirmation of any positive screening outcome is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as required. The entire laboratory method from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
