What it is, How it works

When gathered near the scalp, hair offers an approximate 90-day detection period for alcohol and other substances. Hair samples are easy to gather, hard to tamper with, and simple to transport.

A 1.5-inch section of roughly 200 hair strands (comparable to the diameter of a #2 pencil) nearest to the scalp yields 100mg, the optimal amount for testing and verification. For EtG and tests exceeding 10 panels, a 150mg specimen is advised. We advocate using a jeweler's scale to weigh the specimen. If scalp hair is not available, a similar amount of body hair can be collected. By head hair, we mean only scalp hair. Body hair denotes all other hair (facial, underarm, etc.).

Process Overview

The laboratory procedure for processing a drug test result involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary integration of a sample into a laboratory's system. This step involves confirming that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any additional data entry absent from an electronic chain of custody system.

Screening involves an initial rapid assessment for drugs of abuse. Screening is a cost-effective method to eliminate drug use in most samples; however, a positive screen requires confirmation to be legally valid. Samples that test presumptively positive in Screening must undergo a secondary confirmation.

If a sample is presumptively positive during Screening, additional hair is obtained from the original sample and prepared for Extraction. In this phase, drugs are extracted from hair at significantly lower levels than in other methodologies (such as urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any presumptively positive screening result is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptively positive samples are washed before confirmation if necessary. The entire laboratory process from Accessioning to Confirmation undergoes review under the CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.
