Process Overview

The process of drug test analysis in the lab consists of four key stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, the sample is first entered into the lab’s system. This step includes ensuring the sample's seal and proper shipment, assigning it a Laboratory Accessioning Number (LAN), and entering any additional data missing from the electronic chain of custody.

Screening is a preliminary evaluation for drugs of abuse. This stage is economical for eliminating drug use among most samples, but a positive finding requires confirmation for court validity. Samples testing positive during Screening must undergo a follow-up confirmation.

If a sample shows a presumptive positive in Screening, additional hair is taken from the initial sample for Extraction. This phase involves retrieving drugs from hair in much lower concentrations compared to other methods like urine or saliva, making hair drug analysis particularly challenging.

Confirming a positive screen involves GC/MS, GC/MS/MS, or LC/MS/MS techniques. Any positive samples are cleaned before confirmation when necessary. The entire lab process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair standards and accredited according to ISO / IEC 17025 criteria.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
