What it is, How it works

When harvested near the scalp, hair can yield up to roughly a three-month detection period for alcohol and other substances. Collecting hair is straightforward, relatively tamper-resistant, and easy to transport.

A 1.5-inch portion of around 200 hair strands (akin to the thickness of a #2 pencil) nearest to the scalp will provide 100mg, which is the preferred amount for screening and confirmation. For EtG, additional tests, or panels exceeding 10, 150mg of the sample is advisable. Measuring the specimen on a jeweler’s scale is recommended. If scalp hair is not an option, an equivalent quantity of body hair can be taken. References to head hair pertain exclusively to scalp hair, whereas body hair denotes all other types (facial, axillary, etc.).

Process Overview

The procedure for laboratory processing of a drug test result encompasses four stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a specimen into the lab’s database. It encompasses confirming proper sealing and shipping of the sample, allotting a random LAN (Laboratory Accessioning Number), and fulfilling any additional data input not supplied by an electronic chain of custody system.

Screening entails an initial rapid assessment for illicit substances. While it offers an economical means to dismiss drug use in most cases, court-admissible positive results necessitate validation. Samples indicating a presumptive positive require further confirmation.

If Screening indicates the sample is presumptively positive, additional hair is extracted from the initial specimen and readied for Extraction. At this stage, substances are extracted from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), rendering hair drug screening the most complex method to conduct.

Verification of any positive screening outcome is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive specimens undergo washing before confirmation as necessary. The entire laboratory procedure from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
