What it is, How it works

Hair testing is recognized as an effective means for identifying drug and alcohol usage. Hair retains a comprehensive record of alcohol and drugs by capturing biomarkers within the threads of the hair as it grows. When harvested near the scalp, hair can offer a detection timeframe of up to roughly 3 months for alcohol and other substances. Hair collection is straightforward, tampering is relatively hard, and it's easy to dispatch.

A sample of 1.5 inches consisting of about 200 hair strands (equivalent to a #2 pencil's width) nearest the scalp yields 100mg of hair, which is an optimal sample size for testing and confirmation. For EtG, additional tests, and/or over 10 panels, a 150mg specimen is advised. We advocate measuring the sample on a jeweler's scale. If there's no availability of scalp hair, an equivalent mass of body hair may be obtained. When discussing scalp hair, we mean only scalp hair. Body hair denotes all other hair varieties (e.g., facial, axillary).

Process Overview

The laboratory processing for a drug test result consists of four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of entering a specimen into a laboratory's database. This step includes ensuring the sample is properly sealed and delivered, allocating a unique LAN (Laboratory Accessioning Number), and finalizing any additional data input not supplied by an electronic chain of custody system.

Screening conducts a preliminary scan for drugs of abuse. Although Screening is a low-cost approach to eliminate drug usage in most samples, a positive result from Screening must be validated for it to be recognized legally. Samples testing as presumptively positive require a subsequent confirmation.

If Screening reveals a presumptive positive, more hair is extracted from the first sample for the Extraction stage. Here, drugs are isolated from hair at a much lower concentration compared to other techniques (e.g., urine or oral fluid), which makes hair drug screening the most complex technique to execute.

Verification of any positive Screening result is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation as needed. The entire laboratory workflow from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair classification in addition to the accreditation to ISO / IEC 17025 standards.

Advantages of hair drug testing:

  • Extended detection span: Hair drug tests can identify drug usage for up to 90 days, in contrast to urine tests that have a more limited detection period.
  • Challenging to manipulate: Manipulating a hair drug test is exceptionally difficult, leading to more precise outcomes.
  • Shows past data: It can reveal a timeline of drug use over time, not just recent intake.

Limitations:

  • Cannot identify recent intake: It takes roughly 5-7 days for drugs to be traceable in hair.
  • Expenses: Hair drug tests are usually pricier than other drug testing methods.
  • Result variances: Elements like hair color and individual hair growth variations can affect the levels of drug metabolites found in the hair.

Note: Even though they are often termed "hair follicle tests", the analysis focuses on the hair strand rather than the hair follicle beneath the scalp.
