What it is, How it works

When cut near the scalp, hair can reveal up to a 3-month detection timeframe for alcohol and drugs. Hair collection is straightforward, relatively hard to tamper with, and easy to transport.

A 1.5-inch segment comprising roughly 200 hairs (similar to the girth of a #2 pencil) nearest the scalp will provide 100mg of hair, deemed the optimal sample for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, a 150mg specimen is recommended. Employ a jeweler’s scale for specimen weighing. If scalp hair is unobtainable, an equivalent volume of body hair can be used. By head hair, we refer to scalp hair solely. Body hair encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The pivotal phases in the laboratory analysis of a drug test outcome consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the primary handling of a sample within a laboratory's framework. This step includes validating proper sealing and shipping of the sample, the assignment of a random LAN (Laboratory Accessioning Number), and executing any supplementary data entry not covered by an electronic chain of custody.

Screening consists of an introductory check for drugs of abuse. While Screening serves as a budget-friendly approach to eliminate drug use in the majority of samples, a positive screen necessitates confirmation for legal admissibility. Any samples exhibiting a presumptive positive in Screening mandate a secondary validation.

If a sample demonstrates a presumptive positive during the Screening stage, additional hair is extracted from the original specimen and readied for Extraction. At this juncture, drugs are drawn from hair at concentrations lower than those in alternative methodologies (such as urine or oral fluid), highlighting hair drug screening as the most challenging methodology to execute.

Confirmation of any positive screen result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples undergo washing prior to Confirmation as necessary. The full laboratory series from Accessioning to Confirmation is scrutinized under both CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
