For EtG, additional screenings, or assessments exceeding 10 panels, a specimen of 150mg is advisable. We suggest using a jeweler’s scale to measure the specimen. If scalp hair is absent, a proportionate quantity of body hair may be gathered. References to head hair specifically denote scalp hair. Conversely, body hair encompasses all other hair categories (facial, axillary, etc.).

Process Overview

The comprehensive process in the lab for determining a drug test result encompasses four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the initial stage of integrating a sample into the lab's system. This step ensures the sample was appropriately sealed and transported, allocates a unique LAN (Laboratory Accessioning Number), and finalizes any supplementary data entry absent in an electronic chain of custody system.

Screening involves an initial rapid assessment for substances of abuse. Although cost-efficient, Screening serves to eliminate drug presence in the majority of samples, a confirmed positive screen is necessary to be valid in court proceedings. All samples presumptively positive upon Screening necessitate further confirmation.

If a sample indicates presumptive positivity during the Screening phase, additional hair is obtained from the primary specimen and readied for Extraction. Within this phase, drugs are retrieved from hair at a significantly reduced concentration compared to other methodologies (e.g., urine or oral fluid), underscoring why hair drug screening presents the greatest challenge.

The affirmation of any positive screen result is conducted via GC/MS, GC/MS/MS or LC/MS/MS. Any presumptive positive samples are cleansed prior to confirmation as warranted. The overall laboratory procedure from Accessioning to Confirmation undergoes scrutiny under both the CAP (College of American Pathologists) Hair classification and ISO / IEC 17025 standard accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
