What it is, How it works

Hair analysis stands as a compelling technique for detecting alcohol and drug consumption. Through trapping biomarkers within the hair's fibers, hair offers a lasting record of both alcohol and drug usage. When hair is collected near the scalp, it can grant an approximate 3-month detection period for alcohol and drugs. Hair collection is straightforward, somewhat resistant to tampering, and easy to dispatch.

For testing, a 1.5-inch segment consisting of around 200 hair strands (roughly the diameter of a #2 pencil), gathered nearest the scalp, yields 100mg, the optimal sample size for initial and follow-up tests. For EtG, additional tests, and/or evaluations beyond 10 panels, a 150mg sample is advised. We suggest assessing the specimen's weight using a jeweler’s scale. In the absence of scalp hair, an equivalent quantity of body hair may be utilized. Head hair refers exclusively to scalp hair, while body hair encompasses other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of drug test results involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab's system. This step verifies the proper sealing and shipping of the sample, assigns a random LAN (Laboratory Accessioning Number), and completes any supplementary data entry not covered by an electronic chain of custody system.

Screening entails an initial rapid assessment for drug presence. Although Screening serves as an economical method to exclude drug use in the bulk of samples, a positive screen necessitates confirmation for legal admissibility. Samples that are provisionally positive in Screening require further confirmation.

Should a sample test positive in the Screening stage, more hair is extracted from the initial sample and prepped for Extraction. In this phase, drugs are isolated from hair at lower concentrations compared to other methods (such as urine or oral fluid), which is why hair drug assessment is a challenging methodology.

Confirmation of any positive screening involves techniques like GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples are cleansed prior to confirmation as required. The comprehensive process from Accessioning to Confirmation is scrutinized under the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation standards.

Advantages of hair drug testing:

  • Extended detection span: Hair drug tests can identify drug use for as long as 90 days, in contrast to urine tests which offer a shorter detection span.
  • Challenging to manipulate: Hair drug tests are difficult to falsify, enhancing result reliability.
  • Offers historical insight: It can illustrate a drug use pattern over time, beyond mere recent usage.

Limitations:

  • Incapable of detecting immediate use: Drugs require roughly 5-7 days to become detectable in hair.
  • Expense: Hair drug tests typically cost more than alternative drug testing methods.
  • Result variability: Factors such as hair color and individual hair growth differences can influence drug metabolite concentrations in the hair.

Note: Although commonly termed "hair follicle tests", the evaluation examines the hair strand rather than the follicle beneath the scalp.
