For EtG, additional types, or tests exceeding 10 panels, a 150mg specimen is advised. Using a jeweler’s scale for specimen measurement is recommended. Should scalp hair be unavailable, an equivalent quantity of body hair may be gathered. "Head hair" specifically refers to scalp hair, while "body hair" encompasses all other hair types (such as facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encapsulates four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves the preliminary handling of a sample into a lab’s system. This step includes verifying that the sample’s seal and shipment were adequate, assigning a random LAN (Laboratory Accessioning Number), and completing additional data entry absent from an electronic chain of custody system.

Screening entails an initial swift assessment for abusive drug presence. While Screening is an economically sensible method to dismiss drug use in most samples, a positive screening must be substantiated for legal admissibility. Any samples with presumptive positives in Screening necessitate secondary confirmation.

Should a sample result as presumptively positive during Screening, additional hair is extracted from the initial specimen and prepared for Extraction. This phase involves extracting drugs from hair at concentrations much lower than in alternative methodologies (e.g., urine or oral fluid), rendering hair drug testing the most challenging methodology to execute.

Confirmation of any positive screening is managed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positives undergo necessary washing before confirmation. The complete lab procedure from Accessioning to Confirmation adheres to the CAP (College of American Pathologists) Hair designation and maintains accreditation to ISO / IEC 17025 standards.
