A sample measuring 1.5 inches from about 200 hair strands (comparable to a #2 pencil in size) nearest to the scalp generates 100mg of hair, ideal for both screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, a 150mg specimen is advisable. We suggest using a jeweler’s scale for measuring. If scalp hair isn't available, an equivalent quantity of body hair is acceptable. Head hair refers specifically to scalp hair. Body hair includes all other types of hair (such as facial and axillary hair).

Process Overview

The laboratory procedure for processing a drug test outcome involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the sample's initial incorporation into a lab's system. This step ensures the sample's secure sealing and proper shipment, assigns a random LAN (Laboratory Accessioning Number), and finalizes any extra data input not furnished by an electronic chain of custody framework.

Screening is an initial, swift evaluation for substance misuse. While it is a cost-efficient method to negate drug use for most samples, any positive findings need confirmation for legality in court. Any samples showing a tentative positive in Screening mandate a secondary verification.

If screening suggests a positive result, additional hair is extracted from the primary sample for Extraction preparation. Here, drugs are withdrawn from hair at much lower concentrations than other techniques (e.g., urine or oral fluids), posing it as the most challenging testing methodology.

A confirmed positive screening is verified via GC/MS, GC/MS/MS, or LC/MS/MS methods. All potential positive samples are cleansed before confirmation as required. Both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 standards govern the full lab procedure from Accessioning through Confirmation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
