A 1.5-inch segment of around 200 hair strands (approximately the diameter of a #2 pencil) closest to the scalp provides 100mg, the preferred amount for testing and confirmation. For EtG, additional analyses, and tests beyond 10 panels, 150mg of hair is advised. It’s suggested to weigh the sample using a jeweler’s scale. If scalp hair is not accessible, a comparable volume of body hair can be used. By head hair, we mean only scalp hair. Body hair refers to other types like facial or axillary hair.

Process Overview

The laboratory processing of a drug test involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial step where a sample is logged into the lab's system. This process verifies the sample is properly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and finalizes any additional data input not covered by an electronic chain of custody system.

Screening entails a preliminary assessment for drugs of abuse. While Screening is cost-efficient for eliminating drug usage in most samples, a positive result requires confirmation to be valid in legal settings. Any samples with a presumptive positive result in Screening need secondary verification.

If a sample tests presumptively positive at the Screening stage, additional hair is taken from the original specimen for Extraction. In this stage, drugs are extracted from hair at substantially lower concentrations than in other testing methods (e.g., urine or oral fluid), hence hair drug testing is regarded as the most challenging to carry out.

Confirmation of any positive Screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples that are presumptive positive are cleansed before confirmation as necessary. The entire procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
