What it is, How it works

In the absence of scalp hair, a comparable quantity of body hair can be collected. References to head hair strictly pertain to scalp hair. Body hair encompasses all other hair types, including facial and axillary.

Process Overview

The primary stages of lab processing in drug testing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial integration of a sample into a laboratory’s system. This process involves ensuring that the sample was properly sealed and transported, assigning a random LAN (Laboratory Accessioning Number), and completing the necessary data entry not covered by an electronic chain of custody system.

Screening provides an initial assessment for drugs of abuse. While this method is cost-effective for eliminating drug use in most samples, a positive screening result requires confirmation for legal admissibility. Any samples that yield a presumptive positive during Screening require a subsequent confirmation phase.

Should a sample be presumptively positive during Screening, additional hair is extracted from the original sample and prepared for Extraction. During this stage, drugs are isolated from hair at much lower concentrations compared to other methods (such as urine or oral fluids), which is why hair drug screening presents as the most challenging technique to execute.

Positive screening results are validated through GC/MS, GC/MS/MS or LC/MS/MS Confirmation. Presumptive positive samples undergo washing prior to confirmation when necessary. The entire lab process, from Accessioning to Confirmation, undergoes scrutiny following both the CAP (College of American Pathologists) Hair standards and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
