What it is, How it works

Hair testing has emerged as a formidable means to identify drug and alcohol consumption. Hair records extended drug and alcohol histories by embedding biomarkers within the fibers of the hair strand as it grows. Obtained near the scalp, hair can divulge drug and alcohol use for as long as roughly three months. Hair is easy to collect, challenging to tamper with, and straightforward to transport.

A hair sample of 1.5 inches comprising around 200 strands (about the diameter of a #2 pencil) nearest the scalp will provide 100mg of hair, the preferred quantity for analysis and verification. For EtG, additional checks, or tests exceeding 10 panels, a sample weighing 150mg is advisable. Utilizing a jeweler's scale to weigh the specimen is suggested. If scalp hair is not available, a similar weight of body hair may be utilized. When mentioning head hair, we focus exclusively on scalp hair. Body hair represents all other hair types (such as facial, axillary, etc.).

Process Overview

The core steps in lab processing for drug test outcomes include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses bringing a sample into a lab's system. This step ensures proper sealing and shipment of the sample, assigns a unique LAN (Laboratory Accessioning Number), and finalizes any data entry that an electronic chain of custody system may not provide.

Screening involves a preliminary assessment for abused drugs. While Screening is a cost-efficient method to eliminate drug usage in most samples, a positive screen necessitates confirmation for it to be valid in legal proceedings. Samples with presumptive positives in Screening must undergo additional confirmation.

If a sample shows presumptive positivity in Screening, more hair is extracted from the original sample and prepped for Extraction. Drugs are extracted from hair at significantly lower concentrations than with other approaches (e.g., urine or oral fluid). This is why hair drug screening is the most challenging method to execute.

Confirmation for any positive screening outcome is performed through GC/MS, GC/MS/MS or LC/MS/MS. All samples with presumptive positives are cleaned before confirmation if required. The overall process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair classification and the ISO / IEC 17025 accreditation standards.
