What it is, How it works

Hair analysis has emerged as an effective method for identifying drug and alcohol consumption. Hair can archive a long-term record of substance use by encasing biomarkers within the hair strand as it grows. When gathered near the scalp, hair can offer a detection timeframe of up to around 3 months for alcohol and drugs. Hair collection is straightforward, challenging to tamper with, and easy to transport.

A sample of about 1.5 inches, containing approximately 200 hairs (comparable to the thickness of a #2 pencil) nearest to the scalp, provides 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, ancillary, and/or assessments beyond 10 panels, 150mg of the sample is advised. We suggest measuring the sample with a jeweler’s scale. If scalp hair is not usable, an equal volume of body hair is permissible. By head hair, we mean scalp hair solely. Body hair includes all other types (facial, axillary, etc.).

Process Overview

The four primary stages in the lab processing of drug test outcomes are Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the initial setup of a sample in a lab's system. This step ensures the sample’s seal and delivery are intact, assigns a random LAN (Laboratory Accessioning Number), and completes any needed data entry not covered by an electronic chain of custody protocol.

Screening performs a preliminary rapid test for substances of misuse. Although Screening provides a budget-friendly approach to exclude drug usage on most samples, a positive screening must be validated to be legally acceptable. Any samples that preliminarily test positive in Screening will need a secondary validation.

Should a sample be preliminarily positive in the Screening phase, additional hair is sourced from the original sample for Extraction. In this phase, substances are extracted from hair at substantially lower concentrations than other methods (like urine or saliva), making hair drug screening notably challenging.

The verification of any positive screening result uses GC/MS, GC/MS/MS, or LC/MS/MS methods. All suspected positive samples undergo washing before confirmation as necessary. The entire lab procedure from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 certification standards.
