What it is, How it works

A sample measuring 1.5 inches of approximately 200 hair strands (comparable to a #2 pencil in size) nearest to the scalp yields 100mg of hair, which is the ideal sample quantity for both screening and confirmation tests. For EtG, additional tests, and/or those exceeding 10 panels, 150mg of specimen is advised. Using a jeweler’s scale to weigh the specimen is recommended. In instances where scalp hair is insufficient, a similar amount of body hair can be collected. When head hair is mentioned, it specifically pertains to scalp hair. Conversely, body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result encompasses four primary stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into a laboratory’s system. This stage includes confirming that the sample is sealed and sent correctly, assigning a random LAN (Laboratory Accessioning Number), and finalizing any supplementary data entry absent from an electronic chain of custody system.

Screening provides an initial rapid assessment for drugs of abuse. Although Screening is a cost-efficient approach to eliminate drug usage on most samples, a positive screen requires confirmation for court admissibility. Samples yielding presumptive positive results in Screening necessitate secondary confirmation.

Should a sample appear presumptive positive in Screening, additional hair is extracted from the initial specimen for preparation in the Extraction phase. At this juncture, drugs are extracted from hair at substantially lower concentrations than other methodologies (e.g., urine or oral fluid), which contributes to why hair drug screening is the most intricate methodology to execute.

Confirmation of any positive screening result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation if needed. The comprehensive laboratory process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.
