For EtG, supplementary tests, and/or panels exceeding 10, 150mg of the sample is advisable. Using a jeweler's scale to weigh the sample is recommended. If scalp hair is unobtainable, a similar quantity of body hair can be used. Head hair strictly pertains to scalp hair, whereas body hair encompasses other types (facial, axillary, etc.).

Process Overview

The core aspects of laboratory processing in drug testing include Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into the laboratory system. This step ensures the sample was properly sealed and transported, assigns a random LAN (Laboratory Accessioning Number), and completes any missing information not captured by an electronic chain of custody.

Screening provides a preliminary analysis for drugs of abuse. While Screening offers an economical means to rule out drug consumption in most cases, a positive result necessitates confirmation for legal acceptability. Any samples flagged as presumptively positive during Screening must undergo secondary confirmation.

When a sample is identified as presumptively positive in the Screening phase, additional hair is extracted from the original sample and prepped for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations than with other testing methods (e.g., urine or oral fluid), underpinning the complexity of hair drug screening.

Confirmation of any positive screening outcomes is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptive positive samples require washing prior to confirmation when necessary. The entire laboratory sequence from Accessioning to Confirmation complies with both CAP (College of American Pathologists) Hair specifications and ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
