Hair analysis is increasingly recognized as an effective method for identifying substance and alcohol consumption. Hair captures a chronological record of alcohol and drug use by embedding biomarkers within the strands’ fibers as they grow. When gathered near the scalp, hair offers an approximate 3-month timeframe for detecting alcohol and drug use. Hair collection is straightforward, relatively tamper-resistant, and convenient for transport.
A sample size of about 1.5 inches, comprising roughly 200 hair strands (comparable to a #2 pencil), taken nearest to the scalp, provides 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional testing, or analyses beyond 10 panels, a 150mg sample is advisable. We suggest measuring the sample with a jeweler's scale. If scalp hair is not available, a proportionate amount of body hair may be used. References to head hair specifically denote scalp hair. Body hair encompasses all other types of hair (e.g., facial, axillary).
Process Overview
The laboratory processing of a drug test result entails four key stages: Accessioning, Screening, Extraction, and Confirmation.
Accessioning is the initial integration of a sample into a lab’s system. This process includes confirming proper sealing and shipping of the sample, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data entry missing from an electronic chain of custody system.
Screening serves as a preliminary evaluation for the presence of drugs. While an economical means to dismiss drug usage in most samples, positive screenings require confirmation for legal admissibility. Samples with presumptive positive results from Screening necessitate an additional confirmation.
Should a sample prove presumptive positive during Screening, extra hair is extracted from the original sample for the subsequent Extraction phase. Here, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), rendering hair drug analysis the most challenging method.
Confirmation of any positive screening result is performed via methods such as GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo a washing process prior to confirmation if necessary. From Accessioning to Confirmation, the entire lab procedure is assessed under the CAP (College of American Pathologists) Hair program and ISO/IEC 17025 accreditation.
