The key advantage is the longer detection period, up to 90 days, with observation collection making it hard to falsify results. However, urine tests are still more precise for detecting recent drug use within the last 5 days.

What it is, How it works

Hair testing has gained recognition as an effective method for identifying drug and alcohol consumption. Hair can offer a prolonged history of alcohol and drug presence by embedding biomarkers within the fibers of the hair as it grows. When collected near the scalp, hair may provide up to an estimated 3-month timeframe for detecting alcohol and drugs. Collecting hair is straightforward, somewhat resistant to tampering, and easy to transport.

A 1.5-inch segment of roughly 200 hair strands (approximately the thickness of a #2 pencil) closest to the scalp will yield 100mg of hair, which is the preferred sample for both screening and confirmation. For EtG, supplementary tests, and/or assessments beyond 10 panels, 150mg of the sample is advised. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair isn't available, a comparable amount of body hair can be used. By head hair, we specifically mean scalp hair, while body hair encompasses all other types (such as facial, axillary, etc.).

Process Overview

The laboratory procedure for processing a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial sample processing into the laboratory’s system. This step includes ensuring the sample was sealed and sent correctly, assigning a unique LAN (Laboratory Accessioning Number), and finishing any additional data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is a cost-effective approach for eliminating drug use in the majority of cases, a positive screen must be verified to be valid in court. Samples that test presumptively positive in Screening require further verification.

If a sample tests presumptively positive during Screening, more hair is extracted from the original specimen for Extraction. During this phase, drugs are extracted from hair at far lower concentrations compared to other methods (e.g., urine or oral fluid), which makes hair drug screening the most challenging methodology to execute.

Verification of any positive screening outcome is carried out using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are cleansed before confirmation as needed. The complete laboratory procedure from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and the certification to ISO / IEC 17025 standards.

Note: Although commonly termed "hair follicle tests", the examination actually evaluates the hair strand, not the hair follicle underneath the scalp.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
