What it is, How it works

If gathered close to the scalp, hair offers almost a 3-month timeline for detecting alcohol and drugs. Hair is straightforward to collect, somewhat resistant to tampering, and easy to transport.

A 1.5-inch sample containing roughly 200 strands of hair (around the size of a #2 pencil) nearest to the scalp will yield 100mg, the optimal amount for analysis and validation. For EtG, additional tests, and/or assessments exceeding 10 panels, 150mg is advised. It is suggested to weigh the sample using a jeweler’s scale. If scalp hair is absent, a similar quantity of body hair can be obtained. When referencing head hair, it specifically pertains to scalp hair. Body hair covers all other hair types (facial, axillary, etc.).

Process Overview

The primary stages in the lab analysis of a drug test result consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initiating the sample’s journey into the lab’s system. This step ensures the sample was correctly sealed and shipped, assigns a random LAN (Laboratory Accessioning Number), and completes any extra data entry not covered by an electronic chain of custody system.

Screening is a preliminary fast check for drug abuse. Although Screening offers a cost-efficient way to dismiss drug usage in most samples, a positive screening must be verified to stand in court. Samples that test presumptively positive in Screening necessitate further confirmation.

If a sample shows presumptive positivity during Screening, additional hair is taken from the initial sample and prepared for Extraction. At this phase, drugs are isolated from hair at concentrations notably lower than in other techniques (e.g., urine or oral fluid), which is why hair drug testing is the most intricate method to conduct.

Confirmation of any positive result from Screening is performed via GC/MS, GC/MS/MS or LC/MS/MS. Presumptive positive samples undergo a wash before confirmation as required. The full lab process from Accessioning to Confirmation receives scrutiny under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
