A 1.5-inch segment of around 200 hair strands (comparable in size to a #2 pencil) nearest to the scalp yields approximately 100mg of hair, which is the optimal quantity for both screening and verification. For EtG, additional tests, and panels exceeding 10, it’s advised to gather 150mg of the sample. We suggest using a jeweler’s scale to weigh the specimen. If scalp hair isn’t available, an equivalent quantity of body hair can be gathered. In the context of head hair, only scalp hair is implied. Body hair includes all other hair forms, such as facial and axillary hair.

Process Overview

The four major stages in the lab handling of a drug test outcome are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into the lab’s records. This step involves confirming the sample’s proper sealing and shipment, generating a unique LAN (Laboratory Accessioning Number), and carrying out any remaining data input that isn’t covered by an electronic chain of custody system.

Screening entails an initial rapid evaluation for drug abuse. While Screening is a cost-efficient method to negate drug use in most cases, a positive result from Screening must be authenticated for legal acceptability. Any samples evidencing a presumptive positive during Screening require further confirmation.

If a sample shows a presumptive positive in Screening, additional hair is extracted from the primary sample and prepared for Extraction. Confirmation of any presumptive positive screening result is performed using GC/MS, GC/MS/MS or LC/MS/MS. All samples with a presumptive positive result undergo washing prior to confirmation if necessary. The complete lab process from Accessioning through Confirmation is scrutinized under both the CAP’s (College of American Pathologists) Hair criteria and ISO / IEC 17025 certification standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
