What it is, How it works

Hair analysis is recognized as an effective means for identifying drug and alcohol consumption. As hair grows, it captures biomarkers in its fibers, presenting a substantial timeline of alcohol and drug usage. When sampling hair near the scalp, it can yield a detection window for substances of up to roughly three months. Hair is straightforward to collect, relatively difficult to tamper with, and is easily transportable.

A 1.5-inch segment of roughly 200 hair strands (equivalent to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the perfect sample size for both screening and confirmation procedures. For EtG, add-ons, and/or tests exceeding 10 panels, it is advisable to utilize 150mg of hair. It is recommended to measure the sample weight on a jeweler’s scale. If scalp hair is not present, an equivalent volume of body hair is acceptable. When we mention head hair, it refers exclusively to scalp hair. Meanwhile, body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The four primary steps in the lab processing of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is initially processed into the lab’s system. This stage involves confirming the sample was sealed and transported correctly, assigning a unique LAN (Laboratory Accessioning Number), and finishing any further data entry missing from an electronic chain of custody system.

Screening features a preliminary swift assessment for drugs of abuse. Though Screening offers a cost-efficient method to eliminate drug usage across most samples, positive screens require confirmation to hold up in court. Samples testing as presumptively positive during Screening necessitate a confirmatory testing step.

In instances where a sample tests presumptively positive in the Screening phase, extra hair is extracted from the original sample and prepped for Extraction. Here, substances are extracted from hair at much lower concentrations unlike other methods (e.g., urine or oral fluid), highlighting why hair drug screening remains a challenging procedure.

Any positive outcomes from the screening must undergo Confirmation using GC/MS, GC/MS/MS or LC/MS/MS. Samples that are presumptively positive are cleaned as needed before confirmation testing. The entire lab process from Accessioning to Confirmation adheres to standards from both CAP (College of American Pathologists) for Hair designation and ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
