What it is, How it works

Hair testing is regarded as a robust means for detecting drug and alcohol consumption. Hair offers an extended timeline of drug and alcohol use by capturing biomarkers within the fibers of the growing hair strand. When collected close to the scalp, hair can present up to roughly a 3-month detection period for alcohol and drugs. Hair is straightforward to collect, relatively hard to adulterate, and straightforward to ship.

A 1.5-inch snippet of about 200 hair strands (comparable to the width of a #2 pencil) nearest to the scalp will yield 100mg of hair, the perfect sample size for both screening and confirmation. For EtG, add-ons, and/or tests exceeding 10 panels, 150mg of the specimen is preferred. It's advisable to weigh the specimen with a jeweler’s scale. Should scalp hair be inaccessible, an equivalent amount of body hair might be gathered. When mentioning head hair, we strictly mean scalp hair. Body hair signifies all other hair varieties (facial, axillary, etc.).

Process Overview

The core steps in processing a drug test result in the lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the initial handling of a sample within a laboratory's framework. This involves confirming that the sample was sealed and dispatched correctly, attaching a random LAN (Laboratory Accessioning Number), and completing any additional data entry not offered by a digital chain of custody framework.

Screening pertains to a preliminary rapid check for drug presence. Although Screening offers a budget-friendly approach to negate drug use in the majority of samples, a positive screen requires confirmation to be viable in legal proceedings. Any samples that are tentatively positive in Screening will need secondary confirmation.

If a sample is tentatively positive during the Screening phase, more hair is withdrawn from the original specimen and readied for Extraction. During this stage, drugs are extracted from hair at significantly lower concentrations than in other methodologies (for example, urine or oral fluid), explaining why hair drug screening is the most challenging methodology to administer.

Any positive screening results are confirmed via GC/MS, GC/MS/MS, or LC/MS/MS. All samples suspected of being positive are cleaned prior to confirmation, if necessary. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
