For EtG, additional tests, or panels exceeding ten, a 150mg sample is advisable. We suggest using a jeweler’s scale for weighing. If scalp hair isn’t available, an equivalent amount of body hair can be used. Head hair refers solely to scalp hair, while body hair includes all other types (facial, axillary, etc.).

Process Overview

The four primary stages in processing a drug test in the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning involves initiating the integration of a sample into the laboratory's framework. This process includes verifying the sample's secure sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any necessary data insertion not covered by an electronic chain of custody protocol.

Screening serves as a preliminary rapid check for drug presence. While it serves as a cost-efficient method to exclude drug intake in most cases, a positive screen necessitates confirmation to be court-admissible. Any samples presumed positive during Screening require further confirmation.

Here, drugs are isolated from hair at lower concentrations compared to other methods (such as urine or oral fluid), thus making hair drug screening a more challenging methodology.

Iterative testing of positive results is conducted via GC/MS, GC/MS/MS, or LC/MS/MS. Presumptive positive samples are cleansed before confirmation where essential. The full lab process from Accessioning through to Confirmation gets review under both CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 norms.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
