For EtG, extended tests, and/or testing that exceeds 10 panels, it is advisable to procure 150mg of the sample. Using a jeweler’s scale is recommended for weighing the specimen. In cases where scalp hair is missing, a comparable quantity of body hair is acceptable. "Head hair" denotes only scalp hair, while "body hair" encompasses all other types, including facial and axillary hair.

Process Overview

The laboratory process for handling a drug test result encompasses four fundamental stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails the introductory phase of incorporating a sample into a lab's system. This encompasses ensuring the sample was securely sealed and dispatched correctly, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any additional data input that an electronic chain of custody system might not cover.

Screening is composed of a preliminary, swift assessment for substance abuse. Although Screening is a budget-friendly method to eliminate drug usage for the majority of samples, a positive screen must be confirmed for legal acceptance. Presumptively positive Screening samples necessitate a subsequent confirmation.

If a sample is presumptive positive during the Screening phase, additional hair is extracted from the initial sample for Extraction. In this phase, drugs are isolated from hair at significantly lower concentrations than other methods (e.g., urine or oral fluid), making hair drug screening among the most challenging techniques to execute.

Confirmation of any positive screening results utilizes GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation as needed, all presumptive positive samples are cleansed. The entire lab procedure, from Accessioning through Confirmation, is appraised under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
