A 1.5-inch segment consisting of roughly 200 hair strands (comparable to a #2 pencil's diameter) near the scalp yields 100mg of hair, the optimal sample for analysis and verification. For EtG, supplemental elements, and/or evaluations exceeding 10 panels, a specimen of 150mg is advisable. We advocate weighing the specimen using a jeweler’s scale. If scalp hair cannot be acquired, an equivalent amount of body hair might be used. When referring to head hair, it pertains solely to scalp hair. Body hair denotes any other hair type (facial, axillary, etc.).

Process Overview

Four principal steps comprise the lab processing of a drug test result: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails beginning the sample processing within a laboratory's framework. This involves verifying if the sample was properly sealed and dispatched, assigning a random LAN (Laboratory Accessioning Number), and conducting any supplementary data entry not covered by an electronic chain of custody system.

Screening is a preliminary rapid inspection for substance abuse. Although Screening is a cost-effective method to dismiss drug usage in most samples, a positive result must be verified for legal admissibility. Any samples that test presumptively positive in Screening demand further confirmation.

If a sample is assumed positive during Screening, additional hair is extracted from the initial sample and readied for Extraction. In this procedure, drugs are separated from hair at significantly lower concentrations than other techniques (e.g., urine or oral fluid), which is why hair drug screening represents the most challenging method to execute.

Verification of any positive screening result is performed using GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positive results are cleansed before confirmation if required. The entire laboratory flow from Accessioning to Confirmation is evaluated under the CAP (College of American Pathologists) Hair criterion and the accreditation to ISO / IEC 17025 standards.
