A 1.5-inch segment of roughly 200 hairs (approximating the thickness of a #2 pencil) nearest the scalp will yield 100mg of hair, which is the optimal amount for screening and confirmation. For EtG, additional tests, and/or panels exceeding 10, 150mg of the sample is advisable. Using a jeweler’s scale to measure the specimen is suggested. In the absence of scalp hair, an equivalent quantity of body hair can be obtained. When discussing head hair, we refer exclusively to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result involves four pivotal steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning encompasses the initial integration of a sample into a laboratory’s system. This process entails confirming the sample's proper sealing and shipment, generating a random LAN (Laboratory Accessioning Number), and completing any supplementary data entry absent from an electronic chain of custody system.

Screening constitutes an introductory rapid assessment for drug abuse. While Screening offers a financially efficient means to exclude drug use for the bulk of samples, a positive screen necessitates confirmation for legal admissibility. Any samples yielding a presumptively positive result in Screening necessitate a subsequent confirmation.

Upon obtaining a presumptively positive result at the Screening stage, additional hair is extracted from the initial sample for Extraction. Here, drugs are retrieved from hair at significantly lower concentrations compared to other techniques (e.g., urine or oral fluid), highlighting the complexity of hair drug screening methodology.

Confirmation of any positive screening outcome is conducted via GC/MS, GC/MS/MS or LC/MS/MS. All presumptive positive specimens undergo washing prior to necessary confirmation. The complete laboratory process from Accessioning to Confirmation is thoroughly examined under both the CAP (College of American Pathologists) Hair designation and the accreditation to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
