What it is, How it works

Hair examining has increasingly been recognized as an effective method for identifying drug and alcohol consumption. Hair delivers an extended-term record of alcohol and other substances by capturing biomarkers within the fibers of the growing hair shaft. When collected near the scalp, hair offers up to a roughly 3-month detection timeframe for alcohol and other substances. Hair is straightforward to collect, relatively challenging to tamper with, and simple to transport.

A 1.5-inch section comprising approximately 200 hairs (about the girth of a #2 pencil) nearest to the scalp will produce 100mg of hair, which is the optimal sample for analysis and confirmation. For EtG, additional tests, and/or panels exceeding 10, it is recommended to have 150mg of the sample. We suggest using a jeweler’s scale for weighing the sample. If scalp hair is unavailable, a comparable quantity of body hair might be gathered. When referring to head hair, it pertains solely to scalp hair. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The four key processes in the lab processing of a drug test result consist of Accessioning, Screening, Extraction, and Confirmation.

Accessioning covers the initial processing of a specimen into a lab's system. This involves confirming the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any further data entry not supplied by an electronic chain of custody system.

Screening entails an initial rapid check for substance abuse. While Screening is a cost-efficient means to negate drug use in the bulk of samples, a positive screen must be authenticated to be valid in a court of law. Any samples that are initially positive in Screening require a further verification.

If a sample is initially positive in the Screening step, additional hair is extracted from the original specimen and prepared for Extraction. In this phase, drugs are drawn from hair at a significantly lower concentration compared to other techniques (ex. urine or oral fluid), making hair drug analysis the most intricate methodology to execute.

Verification of any positive screen result is executed via GC/MS, GC/MS/MS, or LC/MS/MS. All initially positive samples are cleansed before confirmation as required. The entire lab process from Accessioning to Confirmation is evaluated under both the CAP (College of American Pathologists) Hair designation and certified to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
