For EtG, added tests, or examinations beyond 10 panels, a sample of 150mg is advised. It is advisable to weigh the sample using a jeweler’s scale. If scalp hair is absent, equivalent body hair may be gathered. In discussing head hair, we mean scalp hair exclusively, while body hair includes all other hair varieties (facial, axillary, etc.).

Process Overview

The process for lab examination of a drug test result includes Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial intake of a sample into a laboratory's workflow. This step ensures that the sample was sealed correctly and dispatched accordingly, assigns a random LAN (Laboratory Accessioning Number), and finishes any extra data tasks not handled by an electronic chain of custody protocol.

Screening comprises an initial cursory evaluation for substances of misuse. Although Screening is economical in excluding drug use for most samples, any positive indications require confirmation for admissibility in court. Samples showing preliminary positives in Screening require further verification.

If a sample shows a preliminary positive in the Screening phase, additional hair is taken from the original sample for Extraction. Here, substances are extracted from hair at a significantly lower level than with other methods (such as urine or oral fluid), making hair drug screening one of the most challenging methods to conduct.

Any positive result from the screening phase undergoes Confirmation with GC/MS, GC/MS/MS, or LC/MS/MS. All samples with presumptive positives undergo washing before confirmation if necessary. The comprehensive laboratory procedure from Accessioning to Confirmation adheres to both the CAP (College of American Pathologists) Hair designation and compliance with ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations. As we embrace these advancements, we pave the way for a safer, healthier society, where drug testing and analysis serve as powerful tools in protecting public health and maintaining integrity in various sectors.
