What it is, How it works

Hair analysis has become recognized as an effective means for identifying drug and alcohol consumption. Hair captures a comprehensive timeline of alcohol and drug usage by securing biomarkers in the fibers of the developing hair strand. When obtained near the scalp, hair can offer an approximately 3-month detection period for alcohol and other drugs. Collecting hair is straightforward, relatively resistant to tampering, and convenient to transport.

A 1.5-inch portion of roughly 200 hair strands (comparable to the width of a #2 pencil) near the scalp will provide 100mg of hair, which is the optimal sample for both screening and confirmation. For EtG, additional tests, and/or examinations exceeding 10 panels, it is recommended to acquire 150mg of the sample. We advise measuring the sample with a jeweler's scale. If scalp hair is not present, an equivalent quantity of body hair can be collected. By head hair, we mean only scalp hair. Body hair encompasses all other types (facial, underarm, etc.).

Process Overview

The key procedures involved in the lab processing of a drug test outcome include Accessioning, Screening, Extraction, and Confirmation.

Accessioning refers to the initial integration of a sample into the lab's system. This covers verifying proper sealing and shipping of the sample, assigning it a random LAN (Laboratory Accessioning Number), and completing any further data entry not included in an electronic custody chain.

Screening signifies a preliminary assessment for potential drug use. Though Screening is cost-efficient for dismissing drug use in the majority of samples, a positive screening must be validated for legal proceedings. Samples that are presumptively positive in Screening require a second confirmation.

If a sample indicates a presumptive positive during Screening, additional hair is extracted from the primary specimen and prepared for Extraction. Here, drugs are retrieved from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug testing one of the most challenging methodologies.

Confirmation of any positive screening is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo washing before confirmation as necessary. The entire lab process from Accessioning to Confirmation is assessed under both CAP (College of American Pathologists) Hair designation and accreditation in compliance with ISO / IEC 17025 standards.

Benefits of hair drug testing:

  • Extended detection period: Hair drug tests can identify drug use for as long as 90 days, unlike urine tests with a shorter detection period.
  • Challenging to manipulate: It is notably challenging to manipulate the results of a hair drug test, ensuring more reliable outcomes.
  • Offers historical insight: It can reveal a usage trend over time, unlike focusing on only recent use.

Drawbacks:

  • Cannot identify recent use: Approximately 5-7 days are needed before drugs become detectable in hair.
  • Expense: Hair drug tests typically cost more compared to other testing techniques.
  • Result variations: Factors such as hair color and personal differences in hair growth can influence the drug metabolite concentration in hair.

Note: Although commonly termed "hair follicle tests", the assessment evaluates the hair strand rather than the follicle beneath the scalp
