What it is, How it works

When gathered near the scalp, hair can furnish approximately a 3-month detection period for alcohol and various drugs. Hair is straightforward to collect, relatively tamper-resistant, and simple to transport.

A 1.5-inch segment of roughly 200 hair strands (comparable to the thickness of a #2 pencil) nearest to the scalp yields 100mg of hair, the optimal sample size for both screening and confirmatory tests. For EtG, additional testing, and/or panels exceeding 10, a specimen of 150mg is suggested. We advise using a jeweler’s scale to weigh the specimen. In the absence of scalp hair, an equivalent quantity of body hair might be used. When discussing head hair, we are specifically referencing scalp hair. Body hair encompasses all other hair types (like facial, axillary, etc.).

Process Overview

The laboratory analysis of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning comprises the initial processing of a specimen into a lab’s system. This step includes ensuring the sample was properly sealed and shipped, assigning a random LAN (Laboratory Accessioning Number), and completing any supplemental entry of data that an electronic chain of custody system did not provide.

Screening consists of a preliminary check for the presence of drugs of abuse. Although Screening is a cost-efficient approach to eliminate drug use for most samples, a positive screening result requires confirmation to be court-admissible. Any samples testing presumptively positive during Screening mandate a secondary confirmation.

If a sample tests presumptively positive at the Screening phase, additional hair is extracted from the primary specimen in preparation for Extraction. During this phase, drugs are isolated from hair at much lower concentrations than other methods (such as urine or oral fluids), which is why hair drug analysis is among the most challenging methodologies.

Confirmation of any positive screening outcome is performed via GC/MS, GC/MS/MS, or LC/MS/MS. Preceding confirmation, all presumptive positive samples are cleaned as necessary. The comprehensive lab process, from Accessioning to Confirmation, is examined under both the CAP (College of American Pathologists) Hair designation and in line with ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
