A sample measuring 1.5 inches with roughly 200 hair strands (about the diameter of a #2 pencil) nearest to the scalp results in 100mg of hair, an optimal specimen for screening and confirmation. For EtG, additional tests, or those exceeding 10 panels, a 150mg specimen is advised. We advocate using a jeweler’s scale to measure the specimen. In the absence of scalp hair, an equivalent volume of body hair may be obtained. When mentioning head hair, we imply scalp hair solely. Body hair encompasses all other types (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result encompasses four steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning entails initially processing a sample into the lab’s system, ensuring proper sealing and shipping, assigning a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry not covered by an electronic chain of custody system.

Screening involves a preliminary assessment for drug abuse. While it is a cost-effective approach to eliminate drug usage in most samples, a positive screen necessitates confirmation for court acceptability. Presumptively positive Screening samples require secondary confirmation.

If Screening indicates a presumptive positive, additional hair is extracted from the original specimen to prepare for Extraction. Here, drugs are isolated from hair at significantly lower concentrations than in other methods (e.g., urine or oral fluid), establishing hair drug screening as the most complex method.

A confirmation for any positive Screening is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples undergo necessary washing before confirmation. The complete lab process from Accessioning to Confirmation is audited under both CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 accreditation.
