An ideal testing sample consists of a 1.5-inch segment from about 200 hair strands (roughly the circumference of a #2 pencil) closest to the scalp, yielding 100mg needed for accurate screening and confirmation. For EtG inclusions or more exhaustive panels, 150mg is advised. We suggest using a jeweler’s scale to measure the sample. If no scalp hair is available, collecting an equivalent volume of body hair is permissible. References to head hair imply scalp hair exclusively. Body hair encompasses all additional hair types (facial, axillary, etc.).

Process Overview

The laboratory procedure for processing drug test results encompasses four pivotal stages: Accessioning, Screening, Extraction, and Confirmation.

During Accessioning, a sample is incorporated into a lab’s system. This stage includes confirming the sample's proper sealing and shipping, assigning it a LAN (Laboratory Accessioning Number) at random, and performing any further data entry not covered by an electronic custody system.

Screening serves as an initial assessment for substance abuse. While cost-effective for most samples, any positive screen requires confirmation for legal credibility. Presumptive positive results in Screening necessitate further confirmation.

Should a sample flag as presumptively positive in the Screening phase, additional hair is extracted from the initial sample for the Extraction phase. Confirmation of positive screening results is accomplished using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with presumptive positives undergo washing before confirmation as necessary. The entire lab procedure, from Accessioning to Confirmation, is scrutinized under both the CAP (College of American Pathologists) Hair designation and ISO/IEC 17025 accreditation standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.

It served as a prominent platform for knowledge exchange and collaboration, facilitating groundbreaking advancements in hair testing and analysis. In this special issue of Drug Testing and Analysis, we aim to showcase the remarkable manuscripts presented at this significant event, shedding light on the latest research and highlighting the future directions in this field.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
