For EtG, additional analyses, and/or evaluations beyond 10 panels, a 150mg sample is advised. We suggest using a jeweler’s scale to measure the sample. If scalp hair is insufficient, a comparable quantity of body hair can be obtained. When mentioning head hair, it pertains specifically to scalp hair. Body hair encompasses all other types of hair (such as facial, axillary, etc.).

Process Overview

The key stages involved in processing a drug test result in a lab include Accessioning, Screening, Extraction, and Confirmation.

Accessioning consists of the primary handling of a sample into the lab's system. This phase includes verification that the sample was securely sealed and shipped, assigning a unique LAN (Laboratory Accessioning Number), and completing any necessary data input not already provided by a digital chain of custody system.

Screening entails a preliminary quick assessment for drugs of abuse. While Screening is a cost-efficient approach to dismiss drug usage in most samples, a positive screening must be confirmed to be acceptable in legal settings. Samples presumed positive in Screening require secondary verification.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original sample and prepared for Extraction. During Extraction, drugs are removed from hair at significantly lower concentrations than other procedures (such as urine or oral fluid), making hair drug screening one of the most challenging methods to execute.

Confirmation of any positive screening results is conducted using GC/MS, GC/MS/MS, or LC/MS/MS. All samples deemed presumptively positive are cleansed before confirmation as necessary. The complete laboratory procedure from Accessioning to Confirmation is assessed under both the CAP (College of American Pathologists) Hair qualification and is accredited to ISO / IEC 17025 standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
