A 1.5-inch portion comprising roughly 200 hair strands (similar in size to a #2 pencil) collected closest to the scalp will provide 100mg of hair, which is the preferred quantity for both screening and confirmation tests. For EtG, supplemental assessments, and/or panels exceeding 10, it is advisable to collect 150mg of the specimen. We suggest using a jeweler’s scale for measuring the specimen. In the absence of scalp hair, a comparable quantity of body hair can be used. The term head hair exclusively refers to scalp hair. Meanwhile, body hair encompasses all other hair types like facial and axillary hair.

Process Overview

The drug test result processing in a laboratory typically involves four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the foundational processing that logs the sample into the lab system. It verifies proper sealing and shipping of the sample, assigns a unique LAN (Laboratory Accessioning Number), and manages any lacking data entry that the electronic chain of custody system doesn’t cover.

Screening performs a preliminary assessment for substance abuse indicators. While Screening is cost-efficient in eliminating drug usage suspicions for the majority of samples, any positive result requires secondary confirmation to hold weight in legal proceedings. Presumptive positive results necessitate further confirmation.

Whenever a Screening stage yields a presumptive positive, additional hair is retrieved from the original collection and readied for Extraction. This stage isolates drugs from the hair at much lower concentrations compared to other methods (such as urine or oral fluid), making hair drug screening notably complex.

Confirmed positive results undergo verification via GC/MS, GC/MS/MS, or LC/MS/MS. Before confirmation, all presumptively positive samples are washed. The entire procedural journey from Accessioning to Confirmation adheres to the standards set by both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
