If scalp hair is unavailable, an equivalent quantity of body hair may be collected. References to head hair denote only scalp hair, whereas body hair encompasses all other types, such as facial and axillary hair.

Process Overview

The drug testing laboratory process comprises four key steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning marks the initial step of processing a sample within a lab's system. This step includes checking the sample for proper sealing and shipping, assigning a unique LAN (Laboratory Accessioning Number), and completing additional data entry that may not be provided by an electronic chain of custody system.

Screening entails a swift preliminary examination for substances of abuse. While this step is cost-effective for eliminating drug use in most samples, any positive result in Screening requires confirmation for court admissibility. Samples that test presumptively positive during Screening need further confirmation.

In cases of presumptive positivity in Screening, more hair is gathered from the original sample and readied for Extraction. During this phase, drugs are extracted from hair at a significantly lower concentration than in other methods (such as urine or oral fluid), which explains why hair drug screening is the most challenging methodology.

Confirmation of any positive screening results is achieved through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are washed before confirmation, as necessary. The entire process, from Accessioning to Confirmation, undergoes review under both the CAP (College of American Pathologists) Hair designation and ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
