What it is, How it works

Hair traps biomarkers within the fibers of the hair strand, offering a historical view of alcohol and drug exposure. When obtained near the scalp, hair can provide roughly a 3-month detection period for alcohol and drugs. It's easy to collect, somewhat resistant to tampering, and simple to transport.

A 1.5-inch section of approximately 200 hairs (around the width of a #2 pencil) near the scalp provides 100mg, which is the optimal quantity for both screening and verification. For EtG, supplemental tests, and/or analyses beyond 10 panels, 150mg of the hair sample is advised. We suggest weighing the sample using a jeweler’s scale. If scalp hair cannot be acquired, a comparable amount of body hair can be used. When we mention head hair, we mean scalp hair only. Body hair includes all other hair categories (facial, axillary, etc.).

Process Overview

The four primary stages in processing a drug test result at the lab are Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial handling of a specimen within a lab’s framework. It involves checking that the sample was properly sealed and sent, assigning a random LAN (Laboratory Accessioning Number), and completing any remaining data inputs not covered by an electronic chain of custody system.

Screening conducts an initial, quick assessment for drugs of misuse. Although Screening is a cost-efficient method for excluding drug presence in most samples, a positive screen must be verified to be valid in court. Any presumptively positive samples from Screening must undergo further confirmation.

If a sample tests presumptively positive during Screening, additional hair is extracted from the original sample and readied for Extraction. At this phase, substances are extracted from hair at a significantly lower concentration than in other techniques (e.g., urine or oral fluid), which makes hair drug testing the hardest methodology to execute.

Confirmation of any positive screen is performed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptively positive samples are rinsed before confirmation, as necessary. The comprehensive lab process from Accessioning to Confirmation is scrutinized under both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
