In the absence of scalp hair, a corresponding quantity of body hair is viable. References to head hair specifically denote scalp hair. In contrast, body hair covers all other types (facial, axillary, etc.).

Process Overview

The laboratory handling of a drug analysis outcome involves four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase of introducing a sample into a laboratory environment. This phase involves ensuring the sample's proper seal and shipment, assigning a random LAN (Laboratory Accessioning Number), and completing any outstanding data entry not available through an electronic chain of custody system.

Screening entails a preliminary rapid assessment for drug presence. Though a cost-efficient approach to largely exclude drug usage in most samples, any positive result in Screening necessitates further validation for legal acceptability. Samples yielding a presumptive positive in Screening require additional verification.

Upon a presumptive positive in the Screening stage, the same specimen undergoes further processing in Extraction. During this phase, drugs are extracted from hair at a lower concentrations than in other methods (e.g., urine or oral fluid), highlighting why hair analysis is among the most challenging methods to execute.

Confirmation of any positive screening result employs GC/MS, GC/MS/MS, or LC/MS/MS techniques. All samples with a presumptive positive result are cleansed before Confirmation as required. Every step from Accessioning to Confirmation is conducted following both the CAP (College of American Pathologists) Hair criteria and the ISO / IEC 17025 accreditation standards.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
