What it is, How it works

Hair testing is recognized as an effective means for detecting substance and alcohol consumption. Hair offers a prolonged record of alcohol and other substances by embedding biomarkers within the strands as they grow. When collected from near the scalp, hair can reveal information on alcohol and drugs up to an approximate 3-month period. Collecting hair is straightforward, somewhat resistant to tampering, and easy to transport.

A sample measuring 1.5 inches, consisting of roughly 200 hair strands (comparable to a #2 pencil), taken from near the scalp provides 100mg of hair, which is perfect for testing and validation. For EtG, additional tests, and/or screenings beyond 10 panels, 150mg of the sample is suggested. Using a jeweler’s scale to weigh the sample is advised. If scalp hair is not present, an equivalent quantity of body hair can be used. When mentioning head hair, we mean scalp hair only. Body hair includes all other hair categories (facial, axillary, etc.).

Process Overview

The lab processing of a drug test result involves four primary steps: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the preliminary processing of a sample into a lab’s system. This process verifies the sample's proper sealing and shipment, assigns a random LAN (Laboratory Accessioning Number), and completes any unprovided data entry from an electronic chain of custody system.

Screening is a fast initial check for drugs. While cost-effective to dismiss drug use in most samples, a positive screening must be confirmed to be legally admissible. Presumptive positive samples in Screening need further confirmation.

If a Screening returns a presumptive positive, additional hair is extracted from the original sample for Extraction. Here, drugs are derived from hair at lower concentrations than other methods (e.g., urine or oral fluid), making hair drug testing more challenging.

Confirmation of a positive screen is done using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed before confirmation if needed. The full lab process from Accessioning to Confirmation is examined under CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
