Collected near the root, hair can reveal up to a roughly 3-month tracking period for drug and alcohol presence. Collecting hair is uncomplicated, relatively hard to tamper with, and simple to transport.

A collection of 1.5 inches comprising about 200 hair strands (approximately as thick as a #2 pencil) nearest to the root provides 100mg of hair, which is the preferred amount for both screening and validation. For EtG, extras, and/or assessments exceeding 10 panels, 150mg of hair is advisable. It's suggested to weigh the sample on a jeweler’s scale. If head hair can't be obtained, a similar volume of body hair is an alternative. References to head hair specifically mean scalp hair. Body hair includes all other types (facial, auxiliary, etc.).

Screening is a thrifty approach to dismiss potential drug usage in most samples; however, confirmation is required for courtroom acceptability if the screen is positive. Presumptive positive outcomes in Screening necessitate further validation.

When a sample is deemed presumptively positive during Screening, further hair is sourced from the initial sample and readied for Extraction. During this phase, substances are extracted from hair at significantly reduced concentrations than those in alternate methods (e.g., urine or oral fluid), explaining why hair drug tests are the most challenging to execute.

Validation of any positive screening finding is performed using GC/MS, GC/MS/MS, or LC/MS/MS. Any presumptively positive samples undergo washing as necessary prior to validation. The complete lab process from Accessioning to Validation is assessed in accordance with both the CAP (College of American Pathologists) Hair standards and the accreditation to ISO / IEC 17025 criteria.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
