What it is, How it works

Hair assessments have emerged as an effective means to identify drug and alcohol consumption. Hair retains a prolonged record of drugs and alcohol by capturing biomarkers within the fibers of the maturing hair strand. When extracted near the scalp, hair offers a detection window of up to roughly 3 months for drugs and alcohol. Hair is straightforward to gather, relatively challenging to tamper with, and simple to ship.

A 1.5-inch segment comprising about 200 hair strands (equivalent to the width of a #2 pencil) nearest to the scalp yields 100mg of hair, which is the ideal quantity for both screening and verification. For EtG, supplementary tests, and/or examinations exceeding 10 panels, 150mg of the specimen is advised. We suggest weighing the sample using a jeweler’s scale. When scalp hair is inaccessible, a similar quantity of body hair can be sourced. The term head hair refers exclusively to scalp hair. Body hair encompasses all other types like facial and axillary hair.

Process Overview

The primary four steps in laboratory processing of a drug testing result include Accessioning, Screening, Extraction, and Confirmation.

Accessioning pertains to the preliminary handling of a sample within a laboratory’s system. This entails verifying proper sealing and shipping of the sample, generating a random LAN (Laboratory Accessioning Number), and concluding any extra data entry omitted from an electronic chain of custody procedure.

Screening entails a preliminary rapid evaluation for drugs of abuse. Screening is an economical method to dismiss drug usage in most samples; however, a positive screen necessitates confirmation to be acceptable in court. Presumptively positive Screening samples require a follow-up confirmation.

In the event that a sample is presumptively positive during the Screening phase, additional hair is extracted from the original specimen for Extraction. During this phase, drugs are drawn from hair at significantly lower concentrations than in alternative methods (such as urine or oral fluid), which is why hair drug screening is the most complex method to execute.

Confirmation of any presumptively positive screen is executed through GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation when necessary. The comprehensive lab process from Accessioning to Confirmation is examined under both the CAP (College of American Pathologists) Hair designation and retains the accreditation to ISO / IEC 17025 standards.

Exposure to drugs was also important in cases of child abuse/neglect, when the clinical purpose of hair testing merge with its forensic applications.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.

In conclusion, the special issue of Drug Testing and Analysis, featuring manuscripts from the SoHT–GTFI meeting in Verona 2022, reflects the remarkable advancements achieved in this field. The manuscripts provide valuable insights into novel analytical techniques, biomarkers, pharmacokinetics, forensic applications, and ethical considerations.
