Process Overview

The laboratory procedures for processing a drug test outcome entail four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the initial integration of a sample into a lab's system. This step involves ensuring the sample was properly sealed and transported, allocating a random LAN (Laboratory Accessioning Number), and completing any additional information input not provided by an electronic chain of custody system.

Screening is an initial rapid assessment for drug presence. While cost-efficient for ruling out drug use in most samples, a positive screen must be confirmed for court admissibility. All samples that screen as presumptively positive require a follow-up confirmation.

If Screening indicates a presumptively positive result, additional hair is extracted from the original sample for further preparation in the Extraction stage. Confirmation of any positive screening is performed using GC/MS, GC/MS/MS, or LC/MS/MS techniques. All presumptive positive samples undergo washing before confirmation as required. The entire lab procedure from Accessioning to Confirmation undergoes review under CAP (College of American Pathologists) Hair designation and accreditation to ISO / IEC 17025 standards.

The joint meeting of the Society of Hair Testing (SoHT) and the Italian Group of Forensic Toxicologists (GTFI) was organized in Verona, June 8–10, 2022, and hosted by the Legal Medicine team of the Verona University Hospital, directed by Franco Tagliaro together with Federica Bortolotti and Rossella Gottardo.
