For EtG, additional tests, and/or tests exceeding 10 panels, a 150mg specimen is advisable. It's recommended to measure the specimen using a jeweler’s scale. In the absence of scalp hair, an equivalent amount of body hair can be gathered. When specifying head hair, it indicates scalp hair solely. Body hair encompasses all other hair types (facial, axillary, etc.).

Process Overview

The laboratory processing of a drug test result consists of four primary stages: Accessioning, Screening, Extraction, and Confirmation.

Accessioning is the initial phase regarding the processing of a sample into a laboratory’s system. This phase includes ensuring the sample was correctly sealed and shipped, allocating a random LAN (Laboratory Accessioning Number), and fulfilling any extra data input not supplied by an electronic chain of custody system.

Screening is a preliminary rapid assessment for drugs of abuse. Although Screening is an economical method to exclude drug use in most samples, a screen showing positive must be validated to be acceptable in legal settings. Any samples indicating a presumptive positive in Screening demand secondary confirmation.

If a sample is considered presumptive positive during Screening, additional hair is extracted from the initial specimen and readied for Extraction. During this phase, drugs are isolated from hair at significantly lower concentrations compared to other methods (e.g., urine or oral fluid), making hair drug screening the most challenging method to execute.

Confirmation of any positive screening result is executed using GC/MS, GC/MS/MS, or LC/MS/MS. All presumptive positive samples are cleansed prior to confirmation when needed. The entire laboratory workflow from Accessioning to Confirmation undergoes review according to both the CAP (College of American Pathologists) Hair designation and the ISO / IEC 17025 standards accreditation.

The legal framework surrounding hair testing and the achievement of guidelines to ensure fairness, accuracy, and confidentiality in drug testing practices have also been discussed,and the SoHT consensus on general hair testing and testing for drugs of abuse was presented at the meeting and published as a Letter to the Editor.
