What it is, How it works

Hair analysis is recognized as an effective method for identifying substance abuse. By encapsulating biomarkers within the strands, hair offers a prolonged record of alcohol and drug consumption. Hair collected near the scalp can yield detection for alcohol and other drugs for up to about 3 months. Collection is straightforward, modification is tricky, and shipment is uncomplicated.

A sample size of 1.5 inches consisting of roughly 200 hair strands (equivalent to a #2 pencil) nearest to the scalp will result in 100mg, optimal for both screening and confirming. For EtG, additional tests, and/or assays exceeding 10 panels, a specimen size of 150mg is advised. Usage of a jeweler’s scale is suggested for specimen measurement. In cases where scalp hair is absent, an equivalent volume of body hair can be utilized. When head hair is mentioned, it pertains only to scalp hair. Conversely, body hair encompasses all other forms (facial, axillary, etc.).

Process Overview

The steps critical to laboratory processing of drug testing are Accessioning, Screening, Extraction, and Confirmation.

Accessioning includes the primary processing of a sample into the laboratory’s workflow. This process entails verifying the proper sealing and shipping of the sample, labeling it with a LAN (Laboratory Accessioning Number), and inputting any supplemental data not captured by an electronic custody chain system.

Screening is an initial, swift assessment for drugs of abuse. Although Screening is a cost-efficient means to exclude drug use in most samples, any positive screen need verification for legal acceptance. Samples testing as presumptively positive at this stage necessitate additional confirmation.

For samples deemed presumptive positive during Screening, further hair is taken from the main specimen and readied for Extraction. In this phase, drug extraction from hair is conducted at substantially lower concentrations compared to other methods (e.g., urine or oral fluid), hence making hair drug testing the most challenging methodology.

Confirmatory analysis for any positive results from Screening occurs via GC/MS, GC/MS/MS, or LC/MS/MS. All samples prone to positive are cleansed as needed before confirmation. The entire laboratory process from Accessioning to Confirmation adheres to CAP (College of American Pathologists) Hair labeling and complies with ISO/IEC 17025 standards for accreditation.

Benefits of hair drug analysis:

  • Extended detection period: Hair drug testing can identify drug use for as long as 90 days, as opposed to urine testing which offers a shorter timeframe.
  • Challenging to falsify: Cheating a hair drug analysis is extremely challenging, resulting in more reliable results.
  • Offers historical insights: It reveals a pattern of drug use over time rather than just recent usage.

Drawbacks:

  • Incapable of detecting recent consumption: Detection typically requires 5-7 days post-drug usage.
  • Expense: Hair drug tests tend to be pricier compared to other testing techniques.
  • Result variability: Conditions such as hair color and individual hair growth variations can influence drug metabolite concentration in hair.

Note: Despite commonly being called "hair follicle tests", the examination focuses on the hair strand rather than the follicle beneath the scalp
